A preliminary report from investigators reveals more about the circumstances surrounding a Colorado rail accident that left a lorry driver dead.

The National Transportation Safety Board (NTSB) has issued its preliminary report into an incident on 15 October in which freight train cars derailed and struck a bridge in Pueblo West (link opens in new tab). The bridge then collapsed, spilling six rail cars on to the highway below. A truck driver was killed in the incident, but the two train crew were unhurt.
